(Unfortunately for Japanese players, the expansion is currently not available in Japan.)
A little background info here: the Shivering Isles is actually the realm of the Daedric Prince of Madness, Sheogorath. So tread its grounds rather lightly and carefully. And take note as well that the expansion allows players to use their already existing characters as well as create new ones.
We've already shown you videos of the expansion and discussed the new achievements so you know that there's a lot to do in the new areas especially within the court of Madness. Aside from the new achievements, Bethesda also incorporated several new in-game content like quests, creatures, weapons, armor, and ingredients.
